Do you have a confirmed diagnosis of SCD?*

Are you 16 years old or older?*

Have you experienced between 2 and 10 VOCs within the past 12 months?*

If you are taking hydroxyurea (HU), L-glutamine, or Oxbryta® (voxelotor), have you been on a stable dose for at least 30 days?*

Are you receiving regularly scheduled red blood cell (RBC) transfusion therapy (also termed chronic, prophylactic, or preventative transfusion)?*

Are you taking or have you received ADAKVEO® (crizanlizumab) within the past 90 days?*
